my desk seems so organized lately all because I haven’t stayed in there for a long time. I’ve been sick for a week now and I can’t drag myself to work and study for our exam : (

I just wish i feel better tomorrow so I can catch up with my readings for Pedia

Happy New Year!

A bit late to greet you all a happy new year. Just an update from me since I haven’t posted in a while…

I dropped two subjects last semester due to my lack of foundation in those two subjects as some of the topics I missed when I got sick with COVID really affected my grades so much that I couldn’t catch up with the load of topics I had to study.

It was a difficult decision to make knowing that I’ll graduate later than my friends and my peers. I know that I had to do it for my future patients. I don’t want to face my patients knowing that I don’t have the knowledge and clinical skills needed to treat them. Despite having to drop 2 subjects, I performed really well with the rest of my courses and Im glad I did what I had to do

Sometimes it’s also important to know when to stop. Stopping at one point does not mean you’re giving up. It only means you’re resting and preparing yourself for another journey to you own success.

At my hospital, physicians MUST make their medical notes accessible to our patients. Problem is, some of the notes are incomprehensible to even to other physicians. I wonder what patients will make of them. (FYI for those not in medicine, S.O.B. in medicine = ”Short/Shortness of Breath”)

A Pacinian Fingerprint at the Scene of the Crime

With all of this criminal activity it’s not surprising the body is full of cells!

A Pacinian or lamellar corpuscle is a mechanoreceptor that is most frequently found at the junction of the dermis/hypodermis in the skin. They are particularly prominent in the skin of the fingers and palms.  In addition to the skin they are also found in other organs (e.g. mesentery, pancreas).

These very unique fingerprint-like structures have characteristic whorls formed from concentric lamella. Each lamella is separated from the next by a fluid filled space. And each lamella is lined on its inner and outer surface by connective tissue synthesized by the fibroblasts also in this region (you can see the nuclei of the fibroblasts in these lamellae in this image).

A nerve runs from the inner core of the corpuscle and is responsible for the transduction of mechanical forces applied to the skin into electrical signals that are conveyed to the central nervous system via the dorsal root ganglion.
This organization of fluid filled lamella and a central nerve are highly sensitive to pressure, particularly vibrational forces generated during tasks that involve a manipulative or tactile nature, like sensing the texture of a surface that is touched.

In fact, the screen of the phone/tablet you are using right now has been designed with the specific intention of stimulating the Pacinian corpuscles in your finger as you scroll.

This is actually a vibrissae hair follicle from the face of a (a whisker).

Vibrissae are sensory hairs that are different from regular body hairs in the fact that they are surrounded by a blood filled sinus (z’s brain) and are associated with sensory neurons that have a distinct and representative pattern in the somatosensory cortex of the mouse brain. The fact that they are well mapped in the brain illustrates their importance in everyday behavior and survival - they are involved in things like detecting, orienting and sensing length of surfaces/objects and tracking (e.g. finding gaps in a maze and answering the question 'can my head fit through here?’).

Note also the sebaceous glands (z’s eyes) which secrete a lipid rich secretion called sebum for maintaining hair and epidermis keratin (keratin is the wispy stuff lining the skin at the bottom of the image and forming z’s lil arms).
The hair in this follicle is absent as you can see z’s wailing mouth is completely empty (where you might expect to see the shaft of a hair).

The closest things us humans have to vibrissae are the thick long hairs in our nostrils which are fairly sensitive to tickles (and make your eyes water if plucked) but more importantly provide a first line of filtration to prevent big particles from being inhaled into your nasal cavity.
